Registration

Please keep in mind that you MUST have the required documents BEFORE your child is entered in the system and placed in a classroom. Parents need to withdraw their child from their previous school and obtain the following documents: Birth Certificate, Social Security card, Immunization on GA Form 3231, and EEDN on GA Form 3300. For students coming from a Georgia public school, your school should have these documents. If you are coming from out of state, the immunization and EED can be obtained through a Georgia doctor’s office or the Health Department. Students in grades 6th through 12th must have a copy of the discipline and attendance for the previous school year and a copy of their transcript. They cannot be scheduled in a class until we have received all records.

You must have a full executed lease, if you are renting, or a loan closing statement, mortgage statement, or tax bill if you are purchasing a home, AND with either of these you must have a current electric bill which shows the service address (GA Power, Mitchell EMC, or Sumter EMC). A copy of your ID is required. You will also be asked to sign an Affidavit of Residency, which can and will be used in a court of law should we find that the information you used to enroll your child was falsely given.

Your previous school might advise that we will send for records, and that you do not need these documents now. As a parent you have rights to these documents so don’t leave without them. We will send for academic records, but the birth certificate, SS card, immunization, EEDN, transcript, and discipline (for 6th to 12th grades) is required BEFORE we can enroll your child. It is the responsibility of the parent to obtain these items for the child they are enrolling.

If you are not the biological or adoptive parent of the student you are enrolling, or if you are a father enrolling your child, be sure and check the required documents for information regarding either of these situations. If you are divorced from the student’s other parent, we must have a copy of the custody portion of your final decree with primary custodial parent living in Lee County.

The district reserves the right to deny enrollment if it feels that falsification of information has taken place or that the student does not reside at the residence stated in the registration paperwork. A home visit may be made if residency cannot be established to the district’s satisfaction. Should the district suspect, or receive information that you do not live where you stated, the district has the right to request further document to establish and prove residency.
