Consent forms have been sent home with students in grades K-5. There will be some available at the upper schools for pick up, or you can print your form from this message or on our district website. Your student's completed form must be turned in by this Friday October 4th if you wish for your student to receive the flu vaccine at school. We cannot accept forms after that date. Please contact your school's nurse with any questions.
